Patrick Airport Transportation Service, LLC d/b/a CHATP Concierge — Version 3.0

1. Independent contractor

You drive as an independent contractor (1099), not an employee. You control your own hours and methods and may drive for others. You're responsible for your own taxes, vehicle expenses, and insurance.

2. Revenue split (85 / 15)

You receive 85% of the fare on each completed trip. The Company retains 15% as a platform and dispatch fee.

3. Platform subscription ($39/month)

A $39.00 monthly subscription keeps you active on the platform and includes your personalized business cards and flyers, your dispatch extension, and your share of the shared advertising fund. It renews automatically until you cancel; cancel anytime and access ends at the close of the paid month.

4. Background check & activation

You authorize the Company and its background-check provider to obtain a consumer report and motor vehicle record (criminal history, driving record, identity) under the Fair Credit Reporting Act. You may receive a copy of any report and dispute its accuracy.

4.3 Activation payment — please read. Your first $39.00 payment activates your application and authorizes the Company to order your background check. Because the Company pays for the check when it is ordered, the first $39.00 is non-refundable once the background check has been ordered, whether or not you are approved.

5. Service standards

Maintain a clean, safe, insured, properly licensed vehicle; arrive on time; treat every guest with courtesy; confirm assigned trips promptly; follow all applicable laws and Company policies, including non-discrimination and service-animal rules.

6. Term & termination

Either party may end this agreement on written notice. The Company may suspend or remove a driver for a failed or expired background check, loss of license or insurance, unsafe conduct, or breach. Governed by Ohio law.
